Disability Forms
Our policy for this medical practice is that Dr. Burgess does not complete disability forms. Therefore, if another medical provider has established that you are disabled and unable to work, they will need to complete the forms certifying your disability. This includes short term disability forms and permanent disability forms.
Family Medical Leave Act (FMLA)
Dr. Burgess is willing to assist in completing FMLA forms if she is the medical doctor providing care for the diagnosed condition on the forms. However, if the primary reason for your FMLA forms is related to care from a Specialist Provider (e.g., cardiology, orthopedics, neurology, etc.), they must be the ones to complete the forms.
Dr. Burgess does not fill out forms on behalf of other medical professionals.
