› How To Prepare For Your Appointment |
The time you have with your doctor is very important to your health. So be sure to ask questions, explain any new symptoms or problems and share your concerns. A basic plan can help you make the most of your appointment whether you are starting with a new doctor or continuing with the doctor you’ve seen for years. The following tips will make it easier for you and your doctor to cover everything you need to talk about. Here are some ways to help you get ready for your next appointment. › Make a list of your questions and concerns Put your most important questions and concerns at the top of your list. This will help the physician learn how to better serve you. › Make a list of all the medications you are taking This includes drugs (prescription and non-prescription), vitamins, and other natural remedies such as herbal products. › Call before your visit to tell the office if you have special needs Ask for an interpreter if you do not speak or understand English well › Ask a friend or family member to come with you Ask a friend or family member to come with you if you think it will be helpful. This person can help listen, take notes and offer support. › Arrive to your appointment 30 minutes early! If it is your first appointment we advise that all patients arrive 30 minutes early › Download and fill out preliminary Registration Form. We advise all new patients to down load the new patient registration forms and complete them prior to their appointment time. › Bring your proof of Insurance With You All patients who wish to use insurance will need to bring their insurance card to all appointments. › Tell us about your doctors and current treatments. Tell your doctor or nurse if you are being treated by other doctors. This includes mental health professionals. |
